Requirements: *High School Diploma or general education degree (GED) required; Bachelor’s degree (B.A.) in Human Services related field; and two years experience working with people with disabilities preferred or equivalent combination of education and experience.
*Must be CPR and 1st Aid Certified.
*Must pass DHS background study.
*Must possess valid driver’s license with acceptable driving record; must be insurable at non-risk rates.
*Must be willing to use personal automobile for company business.
Job Description:

The Individualized Home Support Coordinator assists individuals in developing independent living skills and enhance their level of self-sufficiency.

Primary duties include: Manage and meet weekly with a caseload of individuals receiving individualized home support services. Provide information, coaching and support to assist individuals in achieving their goals in all areas of independent living (rental housing, banking, shopping, budgeting, bill-paying, grocery shopping, community integration, etc.) Assists individuals in identifying resources within the community that fulfill their particular needs. The ideal candidate should be self-motivated, comfortable working independently, have strong interpersonal skills and the ability to communicate effectively and professionally.

Wage: $17.00 per hour
